If your physician does not provide enough information, the coder should NOT guess or randomly choose a highest specificity code.  If the encounter note does not supply the specifics needed you must use the "unspecified" or "NOS" code (NOS stands for Not Otherwise Specified). Try to avoid submitting unspecified/NOS codes as they may be rejected by the payers or you will be paid the lowest amount possible.  A higher specificity code will generally be reimbursed more than the unspecified or NOS code.
