Percentage of patients, regardless of age, who are were prescribed antibiotics or taking natural herbal supplements and undergoing a surgical, therapeutic or diagnostic procedures under anesthesia during the performance period and who have a documented use of a preoperative assessment for anesthesia impairment, documented cross activity and/or contraindication where appropriate AND there was no impairment of the effectiveness of anesthesia Three rates are reported. 1) Percentage of patients who were screened using a pre-operative assessment of antibiotic drug combination or natural/herbal supplement usage 2) Percentage of patients identified with a cross activity or contraindication in the assessment 3) Percentage of patients experiencing no impairment of the effectiveness of anesthesia
